Process for Obtaining Services and Notifying Faculty
Step 1:
Student is referrred (self, faculty, staff, parent, agency) to the Disability Services Coordinator.
Step 2:
Intake meeting is scheduled between Coordinator and student. Interview for services is completed, release of information is signed.
Step 3:
Documentation of disability provided/sought.
Step 4:
Documentaion is reviewed. The professional who wrote the documentation may be consulted. Determination is made regarding reasonable accommodations.
Step 5:
Counselor completes notification form and gives to student to take to appropriate instructors within one week of student registering.
Step 6:
Instructors review faculty notification form. Instructor(s) may confer with student.
Step 7 (if agree):
It is the student's responsibility to introduce him/herself to instructor(s) the first day of class and present the faculty notification form if s/he has not already done so.
Step 7 (if disagree):
Instructor(s) and the Disability Services Coordinator address any difficulties with accommodations. If needed, mediation procedure or appeal follows. Student receives copy of finalized faculty notifcation. (It is the student's responsibility to introduce him/herself to instructor(s) the first day of class and present the faculty notification form if s/he has not already done so.)
